Mental Health Benefits with Modern Health
Modern Health is a mental and behavioral health care platform that you and your dependents (age 6 and above) have access to. Modern Health believes that quick access to personalized mental health support can profoundly impact on your day — whether at home, at work, or in your relationships.
You and each of your dependents (age 6 and above) have access to:
- 8 one-on-one video sessions with licensed clinical therapists
- 8 one-on-one video sessions with certified mental health, professional, or financial well-being coaches
- Unlimited Circles — live and on-demand group support sessions led by Modern Health therapists and coaches that are designed to be safe spaces for sharing and learning with others
- Guided meditations and other programs covering topics like managing stress, parenting, coping with stressful world events, and more
- Ongoing well-being assessments to check-in on your well-being over time
- Access to Care Navigation to guide you through Modern Health benefits
- Provide additional EAP services such as pet resources, childcare support, legal and financial support, leader resources, a rapid response hotline and more

Mental Health Benefits with Moda Health
Employees and family members covered under a Salem Health medical plan have access to many behavioral health benefits and support services through Moda Health. These benefits include, but are not limited to, enhanced benefit coverage for outpatient mental health and substance use disorder office visits.
- Prime Plan – High Deductible Plan (HDP): Cost share is waived for outpatient mental health and substance use disorder office visits after your deductible has been met.
- Choice Plan – Medical Home Plan (MHP): Cost share, including deductible, is waived for outpatient mental health and substance use disorder office visits.

- Behavioral Health: specialty services for managing mental health and substance use disorder benefits. Includes assistance finding in-network providers and understand the mental health and substance use disorder benefits. Includes a dedicated Behavioral Health phone line, 800‐799‐9391
- Comprehensive Mental Health & Substance Use Disorder coverage for members enrolled in the MHP or HDP. Services include:
- Office visits, including psychotherapy
- Intensive outpatient programs
- Coordinated specialty programs
- Transcranial magnetic stimulation and electroconvulsive therapy
- Residential treatment programs
- Substance Use Detoxification
- Health Coaching: A free resource available to all Salem Health employees enrolled in a Moda medical plan. Your coach can provide resources and feedback to help you sleep better, eat healthier and feel your best. Health coaching can also provide you support around stress management and ways to prioritize self-care into your daily routine.

- Cardiac Care, Depression Care, Diabetes Care, Respiratory care and Maternity care
- Case Management: For complex or catastrophic health events and need assistance from a case management registered nurse or behavioral health specialist.

Mental Health Benefits with Meru
Meru Health is a 12 week online mental health support program with a personalized treatment plan with regular communication with a license therapist all accessed from your smartphone with internet access. Over the course of 12 weeks, participants learn and practice skills needed to create long lasting healthy lifestyle habits all with daily support of their dedicated therapist. As lessons are introduced, participants receive messages from their therapist to help them apply the new concepts and build skills.
This evidence-based program includes Motivational Interviewing, Cognitive Behavioral Therapy, Behavioral Activation Therapy, Acceptance and Commitment Therapy, Nutrition Psychiatry, Sleep Hygiene, and more. Ongoing follow up support is provided for weeks 13-52.
Participant details:
- Available to employees and covered family members enrolled in a Salem Health Moda medical plan who are over the age of 18 and live in OR or WA.
- Cost for MHP members: $0
- Cost for HDP members: $0 AFTER met deductible (maximum cost for entire program is $220)
