Simplified Live Well requirements for 2025 benefit year! No wellness exam required before August 2024 to achieve Live Well discount for 2025. The Tobacco-Free and Non-Vaping Affidavit, to be completed during 2025 Open Enrollment, will be the only requirement to qualify for the Live Well discount.
