New Parent/Foster Parent Welcome Kit

A partnership with Amazon Incentives

The New Parent/Foster Parent Welcome Kit program provides each eligible employee an $80.00 Amazon Parenting Essentials Product voucher which can be redeemed for any Parenting Essentials eligible products (found here) or applied toward the purchase of an eligible item over $80.00. ​

To be eligible to receive the Amazon Voucher employee’s must:

  1. Be a regular full or part-time employee of Salem Health Hospitals and Clinics (U-status employees are not eligible at this time)
  2. Have completed their initial introductory period of 6 months.
  3. Complete and submit the New Parent/Foster Parent Welcome Kit application below​
  4. Have a new child added to their family through the following means after November 1, 2020:
    1. Birth
    2. Adoption
    3. Foster placement

How to receive your voucher:

  1. Complete and submit the form linked above.
  2. Login to your Salem Health work email and open the email with your voucher code.
  3. You will receive your voucher code within 10 business days of submission.
  4. Log into you Amazon account or create an Amazon account (no credit card required).
  5. Add the Amazon product voucher claim code to your Amazon account.
    You will now see the total balance on your account information.
  6. Once you are ready to shop, go to:
    “Your Account” → “Your Vouchers” → “Shop for eligible products”

How to purchase eligible products:

  1. You can add eligible items to your cart at any time. Amazon Product Vouchers do not expire.
    1. You can split your product voucher over multiple purchases, combine it with other payment methods and check your balance at any time.
  2. Amazon will apply the product voucher balance to all eligible products. If the balance is insufficient, you will be liable for the remaining balance with a debit or credit card.

Q&A

What if I decide to return an item or cancel my order?

If you choose to return an item or cancel an order paid for with a product voucher, Amazon will refund the balance back to the product voucher – you will not receive the balance back in a gift card or on a debit/credit card.

What if I don’t get my voucher code?

Please submit a request to Human Resources through the AskHR Portal.

What if I have issues with the Amazon voucher once it’s applied to my account?

Please contact Amazon directly at (888) 280-4331.

What if I don’t have an Amazon account?

An Amazon Account is required to use the Amazon Product Voucher. You will be prompted to create an Amazon account and will not be required to provide a credit or debit card.

What if my partner/spouse/ both parents of the child work at the hospital?

Only one product voucher will be provided for each child.

What if I don’t want an Amazon voucher? Is there another option?

Employees are not required to participate in this program. The voucher program is an optional benefit for new parent/foster parents.

Will I be taxed?

Yes – federal regulations stipulate that employee’s pay the required and standard taxation on the face value ($80.00) of this voucher.
