New Parent/Foster Parent Welcome Kit
The New Parent/Foster Parent Welcome Kit program provides each eligible employee with a $80.00 Babylist voucher.
To be eligible to receive the Babylist Voucher employees must:
- Be a regular full or part-time employee of Salem Health Hospitals and Clinics (U-status employees are not eligible at this time)
- Have completed their initial introductory period of 6 months.
- Complete and submit the New Parent/Foster Parent Welcome Kit application
- Have a new child added to their family through the following means after November 1, 2020:
- Birth
- Adoption
- Foster placement
How to receive your voucher:
- Complete and submit the form linked above.
- Login to your Salem Health work email and open the email with your voucher code.
- You will receive your voucher code within 10 business days of submission.
- Log into your Babylist account or create a Babylist account (no credit card required).
- Add the Babylist product voucher claim code to your Babylist account.
You will now see the total balance on your account information. - Once you are ready to shop, go to:
“Your Account” → “Your Vouchers” → “Shop for eligible products”

Q&A
What if I decide to return an item or cancel my order?
If you choose to return an item or cancel an order paid for with a product voucher, Babylist will refund the balance back to the product voucher – you will not receive the balance back in a gift card or on a debit/credit card.
What if I don’t get my voucher code?
Please submit a request to Human Resources through the AskHR Portal.
What if I have issues with the Babylist voucher once it’s applied to my account?
Please contact Babylist directly at (888) 280-4331.
What if I don’t have an Babylist account?
An Babylist Account is required to use the Babylist Product Voucher. You will be prompted to create an Babylist account and will not be required to provide a credit or debit card.
What if my partner/spouse/ both parents of the child work at the hospital?
Only one product voucher will be provided for each child.
What if I don’t want an Babylist voucher? Is there another option?
Employees are not required to participate in this program. The voucher program is an optional benefit for new parent/foster parents.
Will I be taxed?
Yes – federal regulations stipulate that employee’s pay the required and standard taxation on the face value ($80.00) of this voucher.
